SocialOmni: Benchmarking Audio-Visual Social Interactivity in Omni Models

Omni-modal large language models (OLMs) redefine human-machine interaction by natively integrating audio, vision, and text. However, existing OLM benchmarks remain anchored to static, accuracy-centric tasks, leaving a critical gap in assessing social interactivity, the fundamental capacity to navigate dynamic cues in natural dialogues. To this end, we propose SocialOmni, a comprehensive benchmark that operationalizes the evaluation of this conversational interactivity across three core dimensions: (i) speaker separation and identification (who is speaking), (ii) interruption timing control (when to interject), and (iii) natural interruption generation (how to phrase the interruption). SocialOmni features 2,000 perception samples and a quality-controlled diagnostic set of 209 interaction-generation instances with strict temporal and contextual constraints, complemented by controlled audio-visual inconsistency scenarios to test model robustness. We benchmarked 12 leading OLMs, which uncovers significant variance in their social-interaction capabilities across models. Furthermore, our analysis reveals a pronounced decoupling between a model's perceptual accuracy and its ability to generate contextually appropriate interruptions, indicating that understanding-centric metrics alone are insufficient to characterize conversational social competence. More encouragingly, these diagnostics from SocialOmni yield actionable signals for bridging the perception-interaction divide in future OLMs.

SHANKS: Simultaneous Hearing and Thinking for Spoken Language Models

Current large language models (LLMs) and spoken language models (SLMs) begin thinking and taking actions only after the user has finished their turn. This prevents the model from interacting during the user's turn and can lead to high response latency while it waits to think. Consequently, thinking after receiving the full input is not suitable for speech-to-speech interaction, where real-time, low-latency exchange is important. We address this by noting that humans naturally "think while listening." In this paper, we propose SHANKS, a general inference framework that enables SLMs to generate unspoken chain-of-thought reasoning while listening to the user input. SHANKS streams the input speech in fixed-duration chunks and, as soon as a chunk is received, generates unspoken reasoning based on all previous speech and reasoning, while the user continues speaking. SHANKS uses this unspoken reasoning to decide whether to interrupt the user and to make tool calls to complete the task. We demonstrate that SHANKS enhances real-time user-SLM interaction in two scenarios: (1) when the user is presenting a step-by-step solution to a math problem, SHANKS can listen, reason, and interrupt when the user makes a mistake, achieving 37.1% higher interruption accuracy than a baseline that interrupts without thinking; and (2) in a tool-augmented dialogue, SHANKS can complete 56.9% of the tool calls before the user finishes their turn. Continuum: Efficient and Robust Multi-Turn LLM Agent Scheduling with KV Cache Time-to-Live

Agentic LLM applications interleave LLM generation requests with tool calls. These tool calls break the continuity of the workflow by creating pauses between LLM requests, bringing many challenges for the serving system, especially under multi-turn scenarios. Each pause potentially causes KV cache eviction and extra waiting time before entering the continuous batch for the following LLM request. Since these pauses happen for each call, this problem becomes increasingly severe as turn number grow for agentic programs. Previous works either fail to incorporate information from the tool call, evicting KV cache that leads to repetitive prefill or loading, or ignore the continuity of a multi-turn program, creating waiting time between turns that increases per-request latency. We present Continuum, a serving system to optimize job completion time for multi-turn agent workloads by combining tool-aware KV cache timeout with program-level scheduling. By predicting tool call durations in agentic workflows, Continuum selectively pins the KV cache in GPU memory with a time-to-live value based on total turn number. When combined with program-level first-come-first-serve, Continuum prevents scheduling bubbles, preserves multi-turn continuity, and optimizes for throughput for complex agentic workflows. By modeling the variability of tool call and agent program continuity, Continuum outperforms state-of-the-art baselines. Our evaluation on real-world agentic workloads (SWE-Bench and BFCL) with Llama-3.1 8B/70B models shows that Continuum significantly improves the average job completion times, and remains performant across different hardware setups and DRAM offloading schemes. Beyond the Turn-Based Game: Enabling Real-Time Conversations with Duplex Models

As large language models (LLMs) increasingly permeate daily lives, there is a growing demand for real-time interactions that mirror human conversations. Traditional turn-based chat systems driven by LLMs prevent users from verbally interacting with the system while it is generating responses. To overcome these limitations, we adapt existing LLMs to duplex models so that these LLMs can listen for users while generating output and dynamically adjust themselves to provide users with instant feedback. % such as in response to interruptions. Specifically, we divide the queries and responses of conversations into several time slices and then adopt a time-division-multiplexing (TDM) encoding-decoding strategy to pseudo-simultaneously process these slices. Furthermore, to make LLMs proficient enough to handle real-time conversations, we build a fine-tuning dataset consisting of alternating time slices of queries and responses as well as covering typical feedback types in instantaneous interactions. Our experiments show that although the queries and responses of conversations are segmented into incomplete slices for processing, LLMs can preserve their original performance on standard benchmarks with a few fine-tuning steps on our dataset. Automatic and human evaluation indicate that duplex models make user-AI interactions more natural and human-like, and greatly improve user satisfaction compared to vanilla LLMs. Our duplex model and dataset will be released.

When do they StOP?: A First Step Towards Automatically Identifying Team Communication in the Operating Room

Purpose: Surgical performance depends not only on surgeons' technical skills but also on team communication within and across the different professional groups present during the operation. Therefore, automatically identifying team communication in the OR is crucial for patient safety and advances in the development of computer-assisted surgical workflow analysis and intra-operative support systems. To take the first step, we propose a new task of detecting communication briefings involving all OR team members, i.e. the team Time-out and the StOP?-protocol, by localizing their start and end times in video recordings of surgical operations. Methods: We generate an OR dataset of real surgeries, called Team-OR, with more than one hundred hours of surgical videos captured by the multi-view camera system in the OR. The dataset contains temporal annotations of 33 Time-out and 22 StOP?-protocol activities in total. We then propose a novel group activity detection approach, where we encode both scene context and action features, and use an efficient neural network model to output the results. Results: The experimental results on the Team-OR dataset show that our approach outperforms existing state-of-the-art temporal action detection approaches. It also demonstrates the lack of research on group activities in the OR, proving the significance of our dataset. Conclusion: We investigate the Team Time-Out and the StOP?-protocol in the OR, by presenting the first OR dataset with temporal annotations of group activities protocols, and introducing a novel group activity detection approach that outperforms existing approaches. FastSwitch: Optimizing Context Switching Efficiency in Fairness-aware Large Language Model Serving

Serving numerous users and requests concurrently requires good fairness in Large Language Models (LLMs) serving system. This ensures that, at the same cost, the system can meet the Service Level Objectives (SLOs) of more users , such as time to first token (TTFT) and time between tokens (TBT), rather than allowing a few users to experience performance far exceeding the SLOs. To achieve better fairness, the preemption-based scheduling policy dynamically adjusts the priority of each request to maintain balance during runtime. However, existing systems tend to overly prioritize throughput, overlooking the overhead caused by preemption-induced context switching, which is crucial for maintaining fairness through priority adjustments. In this work, we identify three main challenges that result in this overhead. 1) Inadequate I/O utilization. 2) GPU idleness. 3) Unnecessary I/O transmission during multi-turn conversations. Our key insight is that the block-based KV cache memory policy in existing systems, while achieving near-zero memory waste, leads to discontinuity and insufficient granularity in the KV cache memory. To respond, we introduce FastSwitch, a fairness-aware serving system that not only aligns with existing KV cache memory allocation policy but also mitigates context switching overhead. Our evaluation shows that FastSwitch outperforms the state-of-the-art LLM serving system vLLM with speedups of 1.4-11.2x across different tail TTFT and TBT.

Your LLM Agents are Temporally Blind: The Misalignment Between Tool Use Decisions and Human Time Perception

Large language model (LLM) agents are increasingly used to interact with and execute tasks in dynamic environments. However, a critical yet overlooked limitation of these agents is that they, by default, assume a stationary context, failing to account for the real-world time elapsed between messages. We refer to this as "temporal blindness". This limitation hinders decisions about when to invoke tools, leading agents to either over-rely on stale context and skip needed tool calls, or under-rely on it and redundantly repeat tool calls. To study this challenge, we constructed TicToc, a diverse dataset of multi-turn user-agent message trajectories across 76 scenarios, spanning dynamic environments with high, medium, and low time sensitivity. We collected human preferences between "calling a tool" and "directly answering" on each sample, and evaluated how well LLM tool-calling decisions align with human preferences under varying amounts of elapsed time. Our analysis reveals that existing models display poor alignment with human temporal perception, with no model achieving a normalized alignment rate better than 65% when given time stamp information. We also show that naive, prompt-based alignment techniques have limited effectiveness for most models, but specific post-training alignment can be a viable way to align multi-turn LLM tool use with human temporal perception. Our data and findings provide a first step toward understanding and mitigating temporal blindness, offering insights to foster the development of more time-aware and human-aligned agents.

FlowPrefill: Decoupling Preemption from Prefill Scheduling Granularity to Mitigate Head-of-Line Blocking in LLM Serving

The growing demand for large language models (LLMs) requires serving systems to handle many concurrent requests with diverse service level objectives (SLOs). This exacerbates head-of-line (HoL) blocking during the compute-intensive prefill phase, where long-running requests monopolize resources and delay higher-priority ones, leading to widespread time-to-first-token (TTFT) SLO violations. While chunked prefill enables interruptibility, it introduces an inherent trade-off between responsiveness and throughput: reducing chunk size improves response latency but degrades computational efficiency, whereas increasing chunk size maximizes throughput but exacerbates blocking. This necessitates an adaptive preemption mechanism. However, dynamically balancing execution granularity against scheduling overheads remains a key challenge. In this paper, we propose FlowPrefill, a TTFT-goodput-optimized serving system that resolves this conflict by decoupling preemption granularity from scheduling frequency. To achieve adaptive prefill scheduling, FlowPrefill introduces two key innovations: 1) Operator-Level Preemption, which leverages operator boundaries to enable fine-grained execution interruption without the efficiency loss associated with fixed small chunking; and 2) Event-Driven Scheduling, which triggers scheduling decisions only upon request arrival or completion events, thereby supporting efficient preemption responsiveness while minimizing control-plane overhead. Evaluation on real-world production traces shows that FlowPrefill improves maximum goodput by up to 5.6times compared to state-of-the-art systems while satisfying heterogeneous SLOs.

"Even GPT Can Reject Me": Conceptualizing Abrupt Refusal Secondary Harm (ARSH) and Reimagining Psychological AI Safety with Compassionate Completion Standard (CCS)

Large Language Models (LLMs) and AI chatbots are increasingly used for emotional and mental health support due to their low cost, immediacy, and accessibility. However, when safety guardrails are triggered, conversations may be abruptly terminated, introducing a distinct form of emotional disruption that can exacerbate distress and elevate risk among already vulnerable users. As this phenomenon gains attention, this viewpoint introduces Abrupt Refusal Secondary Harm (ARSH) as a conceptual framework to describe the psychological impacts of sudden conversational discontinuation caused by AI safety protocols. Drawing on counseling psychology and communication science as conceptual heuristics, we argue that abrupt refusals can rupture perceived relational continuity, evoke feelings of rejection or shame, and discourage future help seeking. To mitigate these risks, we propose a design hypothesis, the Compassionate Completion Standard (CCS), a refusal protocol grounded in Human Centered Design (HCD) that maintains safety constraints while preserving relational coherence. CCS emphasizes empathetic acknowledgment, transparent boundary articulation, graded conversational transition, and guided redirection, replacing abrupt disengagement with psychologically attuned closure. By integrating awareness of ARSH into AI safety design, developers and policymakers can reduce preventable iatrogenic harm and advance a more psychologically informed approach to AI governance. Rather than presenting incremental empirical findings, this viewpoint contributes a timely conceptual framework, articulates a testable design hypothesis, and outlines a coordinated research agenda for improving psychological safety in human AI interaction.
